ד״ר איילת גולדשטיין בעלת תואר ראשון במדעי המחשב, תואר שני במנהל עסקים שניהם מהאוניברסיטה העברית, ותואר דוקטור בהנדסת מערכות מידע מהאוניברסיטה בן גוריון.
מחקריה של ד״ר גולדשטיין התפרסמו בעיתונים מדעיים בינלאומיים, ועוסקים ביישומי הבינה מלאכותית ברפואה, בפרט ניתוח של כמויות גדולות של נתונים קליניים תלויי זמן.
ד״ר גולדשטיין מרצה בתוכנית לתואר ראשון ושני במחלקה למדעי המחשב במרכז האקדמי הרב תחומי ירושלים.

המחקר שלי מגשר בין שיטות חישוביות מתקדמות לבין יישומים מעשיים בתחום הבריאות. אני מתמקדת בפיתוח מודלים ומערכות פרשניים ומבוססי היגיון קליני, המתרגמים נתונים רפואיים מורכבים וידע הנחייתי (Guidelines) לתמיכת החלטה ישימה ולמדדי איכות מדידים.
1) ניתוח נתונים ולמידת מכונה בבריאות
העבודה שלי מתמקדת בשילוב מעשי של ניתוח נתונים ולמידת מכונה בסביבות קליניות מהעולם האמיתי, במטרה להפיק תובנות בעלות משמעות קלינית מנתונים רפואיים ולתמוך בשיפור אבחון וטיפול.
ציר מרכזי במחקרי הוא פיתוח שיטות מונחות-נתונים לקיבוץ מטופלים ולסיווג רמות סיכון. לדוגמה, פיתחנו גישה רב-ממדית לחלוקת מטופלים עם פיברומיאלגיה לתת-קבוצות על בסיס מגוון פרמטרים בריאותיים, המאפשרת לזהות את הקיבוצים המידעיים ביותר לצורך חקר מצב הטרוגני זה.
דוגמאות כוללות:
1. למידת מכונה באופתלמולוגיה: יישום למידת מכונה לניתוח מדדים של הרשתית ועצב הראייה, כדי לזהות דפוסים הקשורים לפגיעה בראייה ולתמוך בזיהוי מוקדם.
2. מודלים ניבויים באוכלוסיות מגוונות: ניבוי רטינופתיה סוכרתית פרוליפרטיבית (Proliferative Diabetic Retinopathy) באמצעות בדיקות דם ושתן שגרתיות, ובחינה מפורשת של השפעת מין ואתניות על הסיכון למחלה ועל ביצועי המודל, כדי למקד תשומת לב קלינית במטופלים הזקוקים לכך ביותר.
2) מערכות תמיכה בהחלטה קלינית (CDSS) והערכת איכות
אני מפתחת ומעריכה שיטות לתמיכה בהחלטה ולהערכת איכות אוטומטית, באמצעות המרת ידע קליני והנחיות טיפול לייצוגים חישוביים שניתן ליישם בהיקף רחב.
בפרויקט אחד תרמתי לפיתוח מסגרת הוליסטית להערכת איכות טיפול, תוך שימוש בניהול פצעי לחץ כדוגמה מייצגת. המסגרת מקודדת ידע פרוצדורלי של הטיפול לדפוסי הפשטה טמפורלית ומיישמת אלגוריתם של לוגיקה טמפורלית מטושטשת (Fuzzy Temporal Logic) כדי לכמת היענות לפרוטוקולי טיפול ולזהות סטיות בעלות משמעות קלינית.
בפרויקט נוסף סייעתי בפיתוח ארכיטקטורה מבוזרת לתמיכה בהחלטה המדגישה טיפול ממוקד-מטופל, תוך שימוש במכשירים ניידים להעברת הנחיות מבוססות-ראיות בנקודת הצורך, ובכך לתמוך בניהול מחלות כרוניות ובעצמאות המטופל.
כיום אני מנחה סטודנט/ית בפיתוח מערכת CDSS חדשנית אפיזודית ומבוססת הנחיות, שמטרתה לתמוך בקבלת החלטות ותהליכי מעקב לא-רציפים, באמצעות הפעלת המלצות באירועים/אפיזודות קליניים משמעותיים, ולא באמצעות ניטור רציף.
3 ) ייצוג ידע והפשטה טמפורלית בנתונים קליניים
המחקר שלי בניתוח נתונים קליניים הוביל לפיתוח שפת ידע להפשטה טמפורלית (Temporal Abstraction Knowledge Language), שנועדה להמיר נתונים קליניים מורכבים למושגים ברמה גבוהה המתאימים להסקה חישובית.
שפה זו מהווה תשתית למנוע הסקה שפיתחתי, שמטרתו לזהות דפוסים טמפורליים בעלי משמעות קלינית ברשומות מטופלים (למשל מצבים מתמשכים, שינויים לאורך זמן, אירועים חוזרים, וסטיות הקשורות לתזמון). המנוע מתרגם צירי זמן קליניים צפופים ולא-סדירים לייצוגים ברורים ופרשניים, היכולים לתמוך בתמיכת החלטה ובהערכת איכות.
